Quality Cafe: Creating an Effective Aim Statement to Drive Improvement Efforts Event Date and Time Organizer Location
Team-based care is a vital aspect in a growing body of evidence that remission of type 2 diabetes is possible in up to 50% of patients. Several professions fulfill collaborative roles with innovative team-based practices across BC to support patients in their pursuit of type 2 diabetes remission. Join us to hear and discuss how health professionals in two of these practices help patients to achieve shared, measurable goals and outcomes.
The Finding Your Way in Teamwork: Team Coaching Principles Workshop is a two-hour long virtual education session for members of interprofessional rural BC health care teams on the eight elements of high-functioning teams. When teams are able to cultivate these elements they are more able to create a positive work environment, rekindle joy in their work, optimize their team’s performance, and increase the quality and safety of care for patients.
Are you or someone you know interested in learning more about Team Mapping and/ or PACC mapping? Join us for a lunch and learn session on October 12 from 11:30am – 12:15pm to find out more! We will: introduce Team Mapping (what it is, when its useful, how it works), PACC mapping (what it is, when to use it how it works), and provide an overview of ISU resources to support the implementation of Team-Based Care in clinical settings and in communities. Join us on October 19 for the kick off to Season 4 of Team Up! where we will hear about the work that is happening around patient perspectives on the communication of team-based care. Members from UBC Innovation Support Unit, along with patient partners will share their experiences and highlight some key learnings from their current learning cycle, with the goal to inform patient communications around primary care transformation and team-based care.

This 90-minute information session will introduce participants to UBC Health’s Gateway to Team-Based Care project, a multidimensional initiative that focuses on educating health professionals in team-based care with a newly developing primary care clinic at the UBC Vancouver campus. The Gateway to Team-Based Care will support learners from across the health disciplines to work and learn in a team environment with collaborative interprofessional education—envisioned as a cornerstone to support team-based education across British Columbia.
