Submit Team-Based Care Resources
We encourage you to share any resources you think might be beneficial for others on their team-based care journey.
Before being added to this website, all submitted resources will be reviewed by representatives from the Team-Based Care Advisory Group. This review process is meant to ensure that all shared resources are suitable for this setting. Resources will be reviewed for the following criteria where appropriate:
- Overall alignment with the purpose of this website
- Evidence-informed and/or successful application in practice
- Inclusive language used
- Patient partner involvement
- Clarity of information
- Readiness for implementation in practice
