On January 19, 2023 we learned about how CHANGE BC is providing an opportunity for family physicians in BC’s Pacific Northwest to lead and inform the development of innovative Patient Medical Home Teams, with a focus on preventing chronic diseases through primary care lifestyle supports. At this webinar, we heard from this team on their success with this initiative.
On November 17, 2022, we had a panel discussion to learn how providers and coaches in the Sunshine Coast community worked together to generate connections and awareness and develop a shared understanding of the current state of Primary Care Social Work in their region. The discussion focused on the team’s experiences with the utilization of PACC Mapping as a tool to foster conversations between the associated teams and clinicians, with a focus on recruitment and retention challenges, access to care, burden on existing social work services, ideas for futures roles and more.

Before being added to this website, all submitted resources will be reviewed by representatives from the Team-Based Care Advisory Group. This review process is meant to ensure that all shared resources are suitable for this setting. Resources will be reviewed for the following criteria where appropriate:
- Overall alignment with the purpose of this website
- Evidence-informed and/or successful application in practice
- Inclusive language used
- Patient partner involvement
- Clarity of information
- Readiness for implementation in practice
