Team Up! Team-Based Primary & Community Care in Action is a webinar and podcast series that aims to connect individuals and teams, identify tools to apply to current work underway and share experiences in team-based care across the province.

Have you ever wondered what supports are available for teams who are shifting towards interdisciplinary care in BC? Did you know there are a range of coaching supports available – each with a different focus area? Team-based care coaches are vital in setting primary care teams up for success. On February 16, we heard coaches share stories of working with distributed (dispersed) teams across BC. This session explored team development, communication and functional changes that need to happen at the practice level.

Speaker Bios

Lindsay Ferguson

Lindsay is a Provincial Practice Improvement coach focused on Team Based Care with the Practice Support Program within Doctors of BC.  Her principal day to day goals are to guide and support primary care teams, provincial wide, to engage in key conversations, which promote success, from diverse starting points by providing tangible steps and support for all stages thought the implementation and improvement of TBC, from engaging and exploring, preparing, implementing and strengthening Team Based Care.

Lindsay has enthusiastically supported and lead teams in various areas of healthcare as an operational lead and through project management.  She is passionate about the scope of her role and is committed to making change in primary care settings.

Outside of work she can be found hiking with her family and golden retriever, cooking up a storm in the kitchen, and playing boardgames.  Recently, Lindsay became a proud grandma of a little baby boy.

Chelsey McKinney

Chelsey McKinney was born and raised in BC, and is currently living in Kamloops, in the beautiful Interior Region. She has been working in Primary Care since 2011, and has been a part of the Practice Support Program for the last 3.5 years. Her previous experience as an MOA has allowed her to understand the day-to-day challenges that Primary Care practices encounter. Her favorite thing about her role as a Practice Improvement Coach with the Doctors of BC is being able to support providers in building capacity into in their practices by optimizing their EMR, and using data to support and implement proactive care. Outside of work, she spends her time with her family enjoying the outdoors.

Erin Lutz

Erin Lutz is the Manager of Team Based Care Service Delivery for Doctors of BC. In her role, Erin supports the implementation of TBC in the province and works with an amazing team of Team Based Care coaches who work with Primary Care providers and they teams to become high functioning teams. Erin recently completed a Graduate program in Evaluation at the University of Victoria, and is working on finishing a Graduate degree in Community Development focusing on complex system change.

When Erin is not working or studying she can be found driving kids to swimming, running, surfing and gardening.

Christina Thomas

Christina Thomas is the Manager of Health Technology Service Delivery for the Practice Support Program. Christina works with a team of dedicated Health Technology Coaches who provide services to physicians focused on improving the implementation and optimization of health technology. The Health Technology Practice Support Team works as part of the collaborative coaching model alongside the Practice Improvement, Team Based Care and Panel Management Coaches to provide supports to physicians and teams throughout the province.
