Reflections from the Field: Teams Get Real About Team-Based Care (Webinar)
This is the ninth installment of Team Up! Team-Based Primary & Community Care in Action that aims to connect individuals and teams, identify tools to apply to current work underway and share experiences in team-based care across the province.
Organizations and individuals across BC have been hard at work establishing teams in primary and community care to improve access for patients and support high quality care across communities. At this May webinar, participants had the opportunity to hear from primary and community care team members as they share their experiences being part of an interprofessional team and the journey to team-based care.

Erin Berukoff
Erin began her health care career as a Registered Nurse 19 years ago after completing her Diploma in Nursing at BCIT in 2002. She went on to finish her Bachelor of Nursing at UVIC in 2007 and shortly after completed her Emergency Nursing Certificate. Most of her years as a RN where spent on medical/surgical units, ambulatory care and in Emergency. In 2014, Erin graduated from Athabasca University with her Master’s in Advanced Nursing Practice and began her journey as a Family Nurse Practitioner here in Powell River. She has been providing primary health care services to patients at the Family Tree Health Clinic for five years. Erin has recently joined the Regional Leadership Team with the NNPBC Provincial Initiatives Program as the Island Lead.
Erin is a wife, mother to two amazing daughters and a Crossfit enthusiast. She have a new love of horses and at the ripe age of 40 is endeavoring to become a rider on her horse and not just a passenger.
Erin Millar
Since moving to the Okanagan 6 years ago, Erin has had the pleasure of working with the Martin Street Outreach Centre which has just joined the Penticton Urgent and Primary Care Centre. She currently works as the Clinic Support Coordinator and has 11 years’ experience in interprofessional/shared care clinic environments. Erin is driven by her passion for working with vulnerable populations, she takes pride in wrapping care around the patient and meeting them where they are at.
Pamela Toor
Pamela Toor is a Registered Social Worker who is currently the Program Development Manager at the REACH Community Health Centre. In her current role, she supports the development and integration of programs and services supporting the Social Determinants of Health across the REACH clinic and into the community. She has front-line experience working as a social worker as part of the interdisciplinary primary care medical team and is passionate about enacting, empowered, patient-centred care which supports individuals with their mental, physical and social health and well-being.
