Addressing critical incidents in team-based care requires special attention to roles and relationships in the team. The space between professions – communication, processes, role clarification, conflict resolution, collaborative leadership – is integral to quality improvement in team-based care. Join our speakers from an interprofessional team at a Victoria UPCC who will highlight the need for explicit training in team communications amongst primary care providers within a team in order to optimize patient safety.

The formal presentation and question period (60 minutes) will be followed by an opportunity for informal discussion and networking (30 minutes):

SPEAKERS
  • Dr. Thuy-Nga (Tia) Pham, Co-Medical Director, South Island Urgent and Primary Care Centres, Island Health
    Dr Thuy-Nga (Tia) Pham is the co-medical director of the six South Island Urgent and Primary Care Centres (UPCC’s) on Vancouver Island. She remains an Adjunct Professor in the Department of Family and Community Medicine at the Temerty Faculty of Medicine, University of Toronto, where she worked as an academic family physician for close to 20 years, rolling out one of the first interprofessional Family Health Teams in Ontario in 2006 and initiating the Leadership curriculum for the undergraduate medical school. In her current role, she is passionate about expanding the role and scope of the primary care nurse in BC, as well as advancing integration of nurse practitioners into interprofessional primary care teams.
  • Jennifer Matheson Parkhill, RN, Site Director, South Island Urgent & Primary Care Centres, Island Health
    Jennifer Matheson Parkhill is the Site Director for the six UPCC’s in south Vancouver Island. She has been a Registered Nurse since 1989, having spent 13 years as an acute care clinician in medical, surgical and critical care before taking on leadership roles in acute care, primary care, and the Association of Registered Nurses of British Columbia (now Nurse and Nurse Practitioners BC). In her current role, Jennifer is a keen advocate for resources to enable top-of–scope practice for all members of the interprofessional team and for maximizing culturally safe, patient and family centred access for all.
