The Relational Continuity Clinical Practice Guideline helps primary care physicians and the teams they work with to understand the value of relational continuity and adopt practice behaviors to increase it.
The Relational Continuity Change Package accompanies the the Clinical Practice Guideline and serves to expand upon the recommendations in that guideline by identifying actions and tools for practice teams to test.
