Team Up! Social Work and Team-Based Care in the Community – A Dialogue Circle (Webinar)
On November 17, 2022, we had a panel discussion to learn how providers and coaches in the Sunshine Coast community worked together to generate connections and awareness and develop a shared understanding of the current state of Primary Care Social Work in their region. The discussion focused on the team’s experiences with the utilization of PACC Mapping as a tool to foster conversations between the associated teams and clinicians, with a focus on recruitment and retention challenges, access to care, burden on existing social work services, ideas for futures roles and more.
