Join us on November 17, 2022, for a panel discussion to learn how providers and coaches in the Sunshine Coast community worked together to generate connections and awareness and develop a shared understanding of the current state of Primary Care Social Work in their region. The discussion will focus on the team’s experiences with the utilization of PACC Mapping as a tool to foster conversations between the associated teams and clinicians, with a focus on recruitment and retention challenges, access to care, burden on existing social work services, ideas for futures roles and more.

Ava Mundy was born and raised in Sechelt, BC on the traditional territory of the shíshálh Nation. She shares her time between her roles as a Medical Office Assistant and a Practice Improvement Coach with the Doctors of BC Practice Support Program (PSP). She has been working in Primary Care for the last 8 years, and in her role with PSP she has been supporting family doctors and their teams on the Sunshine Coast to implement practice improvements for the last 4 years. Outside of work, she spends her time drinking coffee and tending to her houseplants.
Carleigh Reynolds is a Provincial Practice Improvement Coach focused in Team-Based Care with the Practice Support Program at Doctors of BC. The day-to-day goal is to mentor and support primary care teams across the province to engage in the vital conversations from any diverse starting point, simple to advanced, by providing tangible steps and support for all stages throughout the implementation and improvement of TBC, from engaging and exploring, preparing, implementing, and strengthening and sustaining Team-Based Care.
Carleigh has worked in health care for 28 years and is passionate about Primary Care specifically and how the scope of her role can help support teams.
She is a mother of a busy six-year old. Her time outside of work is spent doing activities with her daughter, including taking photos, going for long forest walks and doing crafts.
Cayce is a registered social worker with the Sunshine Coast Primary Care Network in Sechelt, BC. Working in partnership with the Sunshine Coast Division of Family Practice and local Practice Support Program coordinator, Cayce has developed the PCN social work practice in several family physician clinics.
Cayce is founder and chair of the provincial Social Work in Primary Care committee, and collaborated with Doctors of BC and Ministry of Health to design the first PCN Social Work job description in BC in 2018. He sits as Vice President of the BC Association of Social Workers. For several years, Cayce was Patient Medical Home Project Manager for the Sunshine Coast Division of Family Practice.
In his free time, Cayce laughs and plays with his 2 (soon to be 3) children and wife Maggie. He is an avid hiker and kayaker in Gibsons, and volunteers as parent coach at the local high school.

Wendy’s enthusiastic about the scope of her work each day, and is a lifelong student committed to making change possible and fun one step at a time. She draws in from over 26 years of experience in health care, from acute to coaching in primary care settings and specialized coaching supports in change management and conflict for leadership.
When she is not doing this daytime gig, she can be found hiking, growing flowers, and having many adventures like jumping on the trampoline with her grandkids.
